|
Hidradenitis suppurativa (HS) is a chronic, recurrent inflammatory disease affecting skin that bears apocrine sweatglands. It is difficult to treat medically, especially in severe cases. In this report, the authors present their experienceof an extensive HS case, which was treated surgically. A 49-year-old male patient had an abnormal skin lesion (about20 cm x 10 cm), with many openings and scars on the left buttock. The patient had undergone medical treatmenton many occasions, and other forms of treatment, including incision and drainage, but none prevented recurrence.According to clinical and histopathologic findings, our case was of stage II (Hurley’s staging system), and a surgicalcourse was adopted. Wide surgical excision of all diseased skin and subcutaneous fatty tissue was carried out underspinal anesthesia. The surgical defect was resurfaced with a split-thickness skin graft harvested from the right buttock,immediate postoperative negative-pressure wound therapy was failed because of an incomplete hemostasis. Thesurgical approach chosen resulted in complete healing with no evidence of recurrence at 6 months postoperatively.This report describes a rare case of extensive HS of the buttock, which after wide excision and a split-thicknessskin graft and resulted in a good aesthetic outcome without recurrence.
